BEFORE YOUR SESSION
Client Digital Screening
All clients are required to complete a COVID-19 digital screening questionnaire emailed to you within 12 hours of the appointment time. A new intake form and waiver will also be mandatory for new and returning clients.
Staff Digital Screening & Testing
To minimize the COVID-19 exposure risk to you, our therapists and trainers are required to complete a COVID-19 digital screening questionnaire prior to every shift. Staff also test weekly for COVID-19 using Rapid Antigen Tests.
DURING YOUR SESSION
Please arrive for your session right on time and wait for your appointment time if you arrive early.
Minimal Personal Items
Arrive with minimal personal items and dressed in athletic clothing ready for your appointment. Essential items and coats can be placed in designated gym cubbies or in your treatment space.
Wear A Mask
Face masks are mandatory for all rehab clients coming into the facility. Clients can use cloth face masks. Medical face masks are mandatory to wear for each therapist and trainer.
Hand sanitization for anyone entering and exiting the facility is mandatory. Hand sanitization stations are dispersed throughout the facility.
Therapists and trainers sanitize all equipment and/or the treatment tables in between each appointment. All treatment tables are cleaned with hospital grade disinfectant.
Treatment & Exercise Zones
The clinical space is divided up into specific treatment zones for therapists allowing for physical distancing. The gym space is also divided up into specific exercise zones for trainers allowing for physical distancing.
In an effort to minimize exposure and contact, we’re limiting the use of our change rooms and washrooms. Please arrive at the facility knowing that you may not be able access our washrooms and water fountains during your visit.

DEPTH Training & Physiotherapy Client Vaccination Policy
With the recent announcement made by the Ontario Government regarding mandatory COVID-19 vaccination in non-essential facilities, effective September 22, 2021, DEPTH Training & Physiotherapy (DEPTH) is implementing a policy requiring all members and participants using the gym facilities to be fully vaccinated (including 14 days after final/second vaccination), with the following exceptions:
1. A life-saving emergency;
2. Medical exemptions with appropriate documentation from a medical physician or nurse practitioner (including physician or nurse practitioner’s name and contact information, logo or letterhead identifying the physician/nurse practitioner, statement that there is a medical reason, and a set time period);
3. Children who are 11 years of age or younger (not yet eligible for vaccination);
4. Patients attending essential physiotherapy, massage therapy, or athletic therapy services and not using any of the gym facilities.
If patients are attending physiotherapy, massage therapy, or athletic therapy and need to use the gym in any way, proof of vaccination is required.
You will be asked to provide proof of vaccination upon entering the facility. It will then be checked by our administrative team. To make this process easier for our clients, DEPTH staff will record your vaccination status and when it was checked in our Electronic Medical Records software so you do not need to be checked for vaccination status at each visit. It has been advised that businesses/organizations cannot retain any copies of vaccination information for their customers. All COVID-19 digital pre-screening will remain mandatory before each in-person appointment at DEPTH.
The Ontario Government will be introducing an enhanced digital vaccine receipt that features a QR code, which is safe, more secure, and easier for businesses to verify vaccination status. The new QR based digital vaccination status application should be in place by October 22, 2021. Until such time, you will be required to provide a paper or PDF vaccine receipt that includes all relevant information to prove that you are fully vaccinated.
